An affidavit form can be made under oath or not, depending on your state of residence, county or jurisdiction.

IN ORDER TO WRITE A COMPLETE, RELIABLE AFFIDAVIT FORM YOU MUST:

  • Use a generally accepted format
  • State the facts of the situation or claim in question only, clearly
  • Don‘t allow for any speculation, argument or opinion
  • Write chronologically and unambiguously
  • Anticipate and address any questions that may arise
  • Attach any other documents to serve as proof or evidence for statements made in the affidavit
  • All documents must also be signed, dated and witnessed
  • Tell the whole truth and nothing but the truth

Other names: Affidavit Letter, General Affidavit, Sworn Affidavit, Sworn Statement, Statement Under Oath, Notarized statement, Sworn Oath Statement, Sworn Oath Form... 

It can be used if you are involved in a legal process or be asked to sign it as part of a contract.

Affiant or Deponent - the person making the affidavit.
